WAIVER & RELEASE FORM

In consideration for being accepted as a participant in the Asbury Park Volleyball League and Tournaments, I, agree to assume all risks associated with all events associated with the Asbury Park Volleyball League and Tournaments, and also agree that if I am injured or hurt in any way, that I will not make a claim or bring suit against the Asbury Park Volleyball League and Tournaments, its employees, agents, or any individuals associated with the Asbury Park Volleyball League and Tournaments (hereinafter referred to as the "Releasee"), its representatives, successors and assigns.

In the event that I am injured or hurt in any way during the course of my participation with the Asbury Park Volleyball League and Tournaments, I agree to submit any and all medical claims to my private insurance carrier. In addition, I agree not to make any claim against the Releasee for the loss or damage of personal items which may be related to my participation in the Asbury Park Volleyball League and Tournaments.

I expressly waive all rights for claims and damages which I may have against the Releasee, its representatives, successors and assigns and will hold the Releasee harmless from any and all injuries suffered in connection with the Asbury Park Volleyball League and Tournaments. I know the events and activities offered by the Asbury Park Volleyball League and Tournaments are potentially hazardous. I am in proper physical condition to participate and assume all risks associated with my participation, including, but not limited to falls, contact with other participants, the effect of the weather, including lightning, high heat and/or humidity, all such risks being known and appreciated by me.

I acknowledge that the use of obscene language or inappropriate behavior/misconduct is grounds for termination of my membership and participation with the Asbury Park Volleyball League and Tournaments without a refund. I further acknowledge that I am at least 21 years or older which is the minimum age requirement for participating in the Asbury Park Volleyball League and Tournaments.

I grant the agents of Asbury Park Volleyball League and Tournaments permission to use photographs, video tapes, recordings or any other record of me in this event for any legitimate purpose.

In filling out the form below, I hereby acknowledge I have fully read and fully understand the official registration application and understand my own liability and accept the restrictions described herein.
